With over 4 decades in the industry, our team at Collaborative Community Connections comes together with those individuals placed in our Supportive Living Homes throughout Durham Region and assists these individuals through programming that has been designed to consider their future prospects while respecting their rights and personalities.

Our Residents are encouraged to be involved in our community programming that promotes being productive members and avoids isolation from socially inclusive events.

Our team of seasoned professionals supports Transitioning Adults’ in developing life skills while providing guidance, mentoring, safety and routine.

Our residents are required to follow program guidelines that are based on monitoring health, finances, social law and personal development. While residents will be allowed the freedom to make choices and free of internally set consequences, they will be require to meet certain guidelines in order to maintain their status in our Supportive Living Homes.
